Insurance
We accept most major medical insurance. Insurance benefits will be verified prior to the appointment.
We do NOT accept Medicaid, Tricare, or Humana.
Patients with Out-of-Network coverage may be eligible for Self-Pay rates or possible reimbursement from their insurance companies.
If there are any changes to insurance (i.e. new policy, inactive, etc.), the office must be notified minimum 48 hours prior to the appointment.
